    Please join us on May 12th, 2016 at the Hyeholde Restaurant for the following presentation “Wrong Site Surgery” by Robert Yonash.   Registration will open April 1st, 2016.

    The Pennsylvania Patient Safety Authority: A Resource for Wrong-Site Surgery Prevention

    Since July 2004, more than 700 wrong-site surgery events have been reported through the Pennsylvania Patient Safety Reporting System (PA-PSRS) and analyzed by the Pennsylvania Patient Safety Authority. Of these events, about one quarter have been identified as wrong-site anesthesia blocks administered by an anesthesia provider. Based on these analyses, the Authority has developed evidence-based best practices for preventing wrong-site surgery and wrong-site anesthesia blocks that serve to complement the Joint Commission’s Universal Protocol. To assist Pennsylvania hospitals and ambulatory surgery centers in their efforts to prevent these events, the Authority’s Wrong-Site Surgery Program provides education, tools and resources, and onsite consultation including observation of operating suites followed by a leadership and staff debrief session.

    Robert Yonash, RN, CPPS
    Patient Safety Liaison – Southwest Pennsylvania Patient Safety Authority

    Robert Yonash, a registered nurse, has been with the Patient Safety Authority since 2009 as the patient safety liaison for the southwest region of Pennsylvania. In his role as a Patient Safety Liaison, he is responsible for working with his regional medical facilities to assist them in the reduction and elimination of medical errors. In this position, Robert has been assigned to a number of projects including the statewide CMS Partnership for Patients Hospital Engagement Network Wrong Site Surgery Prevention Collaboration and the Pennsylvania Patient Safety Authority/Pennsylvania Society of Anesthesiologists Wrong-Site Block Initiative. Robert is a member of the American Society of Professionals in Patient Safety and has attained certification as a Certified Professional in Patient Safety (CPPS) and Certified Six Sigma Lean Green Belt in Healthcare. He is also a master trainer in TeamSTEPPS.

    Before coming to the Authority, Robert worked as a Health Facility Quality Examiner (Surveyor) in the Pittsburgh Field Office for the Pennsylvania Department of Health Division of Acute and Ambulatory Care. He also held a similar position with the Department of Health Division of Nursing Care Facilities. In these positions he was charged with surveying a range of healthcare facilities including hospitals, ambulatory surgical facilities and nursing homes for compliance with state and federal (CMS) regulations as well as federal organ transplant regulations.

    In his other experience he worked at the Monongahela Valley Hospital in the risk management/quality improvement department. In his role with the hospital, he aggregated and analyzed data for quality, performance improvement, patient safety and risk management. He also worked with numerous committees on various patient safety issues including: patient falls, patient restraints, resuscitation review, medication errors and adverse drug reactions.

    Robert is a graduate of Penn State University and has held numerous registered nursing positions in a number of hospitals and nursing homes in southwestern Pennsylvania.
